Here's a Ray Allen Celtics I had done.
I send all of my jerseys-- hockey, baseball and basketball-- to them. It's expensive, but, in my experience, they always do it right.

I'd just send it to EPS or a similar uniform customizer.

Last week I went to a Marshalls and surprisingly found a Mitchell & Ness 94-95 NBA All-Star Scottie Pippen jersey. After looking it over to make sure it wasn't a fake (I was fooled once before with a Magic Johnson jersey) I noticed something very odd on the label above the Mitchell & Ness patch at the bottom left of the jersey. Instead of reading "1994-1995 Scottie Pippen", it instead reads "1988-1989 Dennis Rodman".[/font][/font]
[font=Arial,Helvetica][font=Arial,Helvetica]I actually contacted Mitchell and Ness and showed them detailed images. They confirmed that it was real and that they "switch the labels from time to time". I'm assuming that means they make mistakes occasionally.
