Well, the main reason we don't see it as much is because the NBA has re-tooled the lottery process many times over to prevent such things from happening. The first few years, every non-playoff team had an equal shot. Then, they went to a weighted lottery, but the Magic won two years in a row, so they increased the odds for bad teams while decreasing odds for better teams. They've been tinkering with it ever since.
Also, your Tim Duncan example is bad because the Spurs had the third best odds that year. They didn't jump like, say, the Bulls in 2008, who had the ninth worst record and a 1.7% chance at the #1 pick.
