Looks like a modified version of the Decline lockscreen that comes with MIUI.

Go to Data, Widgetlock folder, themes...Drop the .zip files in there

also if its a rar rename it mtz? or is that just for complete themes?

thanks b. took me 2 hours to place em somewhere.

IYE2- there's already a complete MIUI theme with his icons in it. put it in your sd card/MIUI/themes and thats it, look for it in theme manager and upload it.
