In the 'Tarkin' book, it's revealed that he has very very high suspicions that he's Anakin. But it's never revealed if he takes it as a fact or not.

I always assumed he did. The guy was literally #2 to Palpatine in ANH, surely he figured it out in the 20 years between ROTS and ANH.
