Honestly, If I was Bruce Bochy and saw Casilla swing at the 3-2 pitch (that he fouled off), I would have called timeout and gone out and reprimanded him.  If Bochy reeeeaaaally told Casilla he didnt want him to swing or run before his at bat, I find it unacceptable that he allowed Casilla to swing at multiple pitches.
