It's implied that she was killed in the books but GRRM didn't explicitly state how.

I watch with captions the first time around so I don't miss anything, and as soon as the band started playing and it said "band playing reigns of castamere" I knew it was about to go down...

They're cursed. The Starks are like the Kennedys of Westeros. Don't forget that Ned's brother and father were burned alive. His sister also passed away too soon. Never heard what happened to his mother, she might've died brutally for all we know too.Damn! Starks stay losing
