Hyperbolic time chamber when training with gohan. It's during the same time that trunks uses the form against cell and both characters explain the downside of bulking up like that.

Fusion has always been weird to me. They're definitely setting super up for an epic return to fusion with goku and vegeta. Wouldn't be surprised if we see it as soon as the tourney arc

They would likely die as I remember the old Kai saying something about how Goku shouldn't go Super Saiyan after Potara fusing with Vegeta as it would **** up his lifespan

I'm aware that Akira Toriyama was extremely forgetful but SS2 def appeared multiple times in the buu saga. By goku, Gohan, and most of all vegeta. Maybe he forgot about it in other media? He probably forgot about it after DBZ had been completed.
Most notably The goku vs Majin vegeta fight, arguable one of the best fights in DBZ, was entirely in SSJ2.
