In order to design+build higher than two floors you require an Architect. That means production-style of construction is gone and you must build under the "Custom Home" delivery system. Me? Start making houses with two-level basements.
Why would people pay extra just to have an extra floor below ground when they could have it above ground for cheaper?
It was only about 50 yrs ago that we didn't even utilize the basement for living accommodations, and now it's standard practice to include the basement as square footage.
The only complication I could foresee is the complication of having no windows... not sure if that's allowed. You can't exit easily if there is a fire. Okay, turns out it's a ++!$+$ idea

Excavation ain't cheap. Would it be that much cheaper than building a 3rd floor or just adding square footage to the existing floor plan?
Just seems there are easier ways to add square footage than adding a second floor underground.
