depends on the pitcher. and it's to make pitchers work a bit more

Lots of variables. 2 most common is first at-bat of the game and new pitcher in relief.

It used to be the leadoff hitter wasn't just a great on base guy, but if they could run the count full, even if they got out, it was a good thing because typically he's used to communicate to the dugout what the pitches are doing because you'd see the pitcher go thru his repertoire trying to get the out (fastball running?, Breaking ball flat?, etc.) . There's a lot more first ball fast ball hitters now though it seems like. Analytics play into it.
 
Giolito turned into buns quick. He was pretty good a couple seasons ago
